You'll need

1. To exclude yourself from the Settlement, you must complete and sign an opt-out request. (Deadline: June 2, 2026)
2. Settlement Class Members submitting claims for Ordinary Loss Relief and/or Extraordinary Loss Relief must submit documentation and attestation supporting their claims.
3. Settlement Class Members must also include an attestation that the monetary losses are fairly traceable to (for Ordinary Loss Relief) or caused by (for Extraordinary Loss Relief) the Incident and were not incurred due to some other event or reason.
4. Submit your Claim Form by July 2, 2026 to receive payment from the Settlement.
5. To object to the Settlement, submit a written objection to the Court (with copies to Class Counsel and Defendants' Counsel) postmarked by June 2, 2026.
