You'll need

1. To exclude yourself from the Settlement, you must submit an opt-out request no later than June 23, 2026.
2. Your objection must be submitted to the Claims Administrator by no later than June 23, 2026.
3. Submit your Claim Form by July 23, 2026 to receive payment from the Settlement.
4. You must submit your Claim Form online no later than Thursday, July 23, 2026, or mail your completed paper Claim Form so that it is postmarked no later than Thursday, July 23, 2026.
5. You must complete and mail your written request for exclusion so that it is postmarked no later than Tuesday, June 23, 2026.
6. You must file your objection(s) and/or notice of intent to appear at the Final Approval Hearing so that it is submitted or postmarked no later than Tuesday, June 23, 2026.
