You'll need

1. You must complete a Claim Form and mail it to the Notice and Claims Administrator so that it is received no later than July 10, 2026, or you can submit your claim online no later than July 10, 2026
2. Transaction data is mandatory for claims of $100,000 or more, although the Claims Administrator may also require transaction data for claims of less than $100,000, so keep related transaction data and any other documentation supporting your claim in case the Claims Administrator requests it later.
3. If your claim is for less than $100,000, you should still provide the transaction data with your claim submission if you can.
4. you must e-mail the Claims Administrator by July 10, 2026, or send a letter to the Claims Administrator by first-class mail postmarked no later than July 10, 2026, stating that you want to exclude yourself.
5. You must e-mail your comment or objection to the Claims Administrator by July 10, 2026, or send via first-class mail your comment or objection to the Claims Administrator postmarked no later than July 10, 2026.
6. Settlement Class Member on an individual basis, and any request for exclusion by a purported authorized agent or representative of a Class Member must include proof of the representative’s legal authority and authorization to act and request exclusion on behalf of each Class Member they seek to opt out. (Deadline: July 10, 2026)
7. Class Member for the purpose of making comments or objections must also file a Notice of Appearance with the Court using the Court’s Case Management/Electronic Case Files (CM/ECF) system by no later than July 24, 2025 (see Question 25).
