- To file a claim for Out-of-Pocket Losses, Lost Time, Alternative Cash Payment, and/or Identity Defense Services, you must submit a Claim Form by August 14, 2026.
- To opt out of the Settlement, submit a written Request for Exclusion to the Settlement Administrator postmarked by June 1, 2026.
- To object to the Settlement, submit a written objection to the Court (with copies to Class Counsel and Defendants' Counsel) postmarked by June 1, 2026.
